Arbiter Sports & Paperwork

 

Arbiter Sports registration, the Medical Eligibility Form/ Health History Update Form, Random Drug/Alcohol Testing Form, and concussion baseline testing (when applicable) is required to participate in Hackettstown High School athletics. 

^ Must be Completed BEFORE JULY 28th, 2025

(Participation will be delayed until all paperwork, registration, and testing are completed).


STEP 1: Register on Arbiter Sports
  • This needs to be COMPLETED FOR EACH NEW SEASON
  • Arbiter Sports is a user-friendly way to register for our programs as a way to keep track of policies and medical information. 
  • We highly suggest registering the student-athlete as early as possible if they are not registered we don’t know they are interested in participating. As long as the student-athlete is registered to participate we can communicate before paperwork deadlines as the season gets closer to starting. 
  • Once the student-athlete's registration has been reviewed you will receive an email, to the account you registered with, that it has been either "accepted" or "not accepted".
    • "Accepted" means the student-athlete has all the required medical paperwork to participate.
    • "Not Accepted" means there is something missing with the student-athlete's paperwork and the email you receive back will explain what is missing. 
  • If you need assistance with logging in, please call  Arbiter Sports 888-800-5583 ext: 1. Unfortunately the school cannot assist with log in/password issues. We have noticed the website is more user friendly on a computer/laptop instead of a smartphone. 
 
Fall Sports: Registration Open NOW. 
STEP 2: Medical Eligibility Form
  • This form is the final page of the"Preparticipation Packet" aka the packet of paperwork that is brought to the physician's office at the time of their yearly physical.
  • A Medical Eligibility Form is required to participate in all sports, color guard, and marching band. This needs to be COMPLETED EACH YEAR the student-athlete participates.
  • This is the only form within the Preparticipation Packet that should be returned to the school. All other paperwork should be kept with the doctor or with your personal files. 
(and/or) STEP 2: Health History Update Form
  • This form applies to any student whose physical was completed more than 90 days prior to the first official practice and we already have a Medical Eligibility Form for them on file at the school.
  • It is completed by parents/guardians and NOT by the physician who completed the physical. 
  • The Health History Update Form needs to be COMPLETED FOR EACH NEW SEASON.
STEP 3: Concussion Baseline Testing
  • The 2025-2026 school year will be the first year Hackettstown High School will be utilizing new, more accurate software. Because the software is new, all students participating in the sports below will need a new test.
  • Typically this test will be done their freshman year and their junior year. We will keep current students on a 2 year rotation and all incoming students will take the test their freshman and junior years. 
  • More information about the test itself will be available shortly under the Athletic Training tab.

  • This is required for the following sports...
  • Fall: Cheerleading, Field Hockey, Football, Boys Soccer, Girls Soccer, Volleyball.
  • Winter: Boys Basketball, Girls Basketball, Cheerleading, Ice Hockey, Wrestling
  • Spring: Baseball, Boys Lacrosse, Girls Lacrosse, Softball
  • Any athlete participating in a sport that the testing is not required can take a baseline concussion assessment if they would like. 
STEP 4: Random Drug/Alcohol Testing Form
  • The Random Drug Test form is required to participate in all sports, marching band and color guard.
  • It can be handed in with the Medical Eligibility Form or to the guidance office, as they are the ones who process this form. 
  • This form only needs to be completed ONE TIME during your entire time participating in Hackettstown High School sports. 

The Medical Eligibility Form, Health History Update Form, and Random Drug/Alcohol Testing Forms should be printed out and handed in to the athletics office during school hours or can be dropped off at the main office of the high school (or mailed) addressed to Shea Hannigan.
