Arbiter Sports & Paperwork

Arbiter Sports registration, the Pre Participation Packet/Updated Health History form, Random Drug Test form, and concussion baseline testing (when applicable) is required to participation in Hackettstown HIgh School athletics. 
(Participation will be delayed until all paperwork, registration, and testing are completed).
Arbiter Sports (formerly known as Family ID)
  • Arbiter Sports is a user-friendly way to register for our programs as a way to keep track of policies and medical information. 
  • This needs to be COMPLETED FOR EACH NEW SEASON
  • We highly suggest registering the student athlete as early as possible. As long as the student-athlete is registered to participate we can communicate before paperwork deadlines as the season gets closer to starting. 
  • Once the student-athlete's registration has been reviewed you will receive an email, to the account you registered with, that it has been either "accepted" or "not accepted".
    • "Accepted" means the student-athlete is cleared to participate.
    • "Not Accepted" means there is something missing with the student-athlete's paperwork and the email you receive back will explain what is missing.

Pre Participation Packet
  • A physical is required to participate in all sports, color guard, and marching band. This needs to be COMPLETED EACH YEAR the student-athlete participates.
  • Physicals are only valid for 365 days from the date the physical was performed. (For example: a physical performed on July 2, 2024 would be valid until July 1, 2025).
  • Physicals done by a private physician have to be reviewed by the school's physician before the student athlete can participate, as required by the State of New Jersey. 
      • If a physical is done by a private physician it must be stamped by the office it is completed at to be valid.
      • The private physician must have completed the "Cardiac Assessment Professions Development Module" and will need to date the physical with the date they completed that module on your student-athletes' Pre Participation Packet. 
  • Physicals must be submitted only on the state forms provided. If they are not on the specified paperwork they will not be accepted. 
  • Please make sure all applicable questions, signatures, and stamps are completed before handing it in. Student-athletes are required to sign the forms as well.
  • Hackettstown High School hosts a FREE physical date for all students interested in participating in any sport for the 2024-2025 school year, in any season, on Saturday June 15th. Please see the athletics homepage for more information. We highly suggest students to attend, as this ensures their physical is valid for the entire following school year. 
Health History Update Form
  • This form applies to any student whose physical was completed more than 90 days prior to the first official practice. 
  • It is completed by parents/guardians and NOT by the physician for completed the physical. 
  • The Health History Update Form needs to be COMPLETED FOR EACH NEW SEASON.
Random Drug Test Form
  • The Random Drug Test form is required to participate in all sports, marching band and color guard.
  • It can be handed in with your Pre Participation Packet.
  • This form only needs to be completed ONE TIME during your entire time at Hackettstown High School. 
All packets and forms should be printed out and handed in to the athletics office during school hours or can be dropped off at the main office of the high school (or mailed) addressed to Shea Hannigan.
